PD Connect® is an essential program designed to empower healthcare professionals in British Columbia by providing them with an easy way to refer individuals with Parkinson’s disease, as well as their families, to Parkinson Society British Columbia’s support services. Whether at the time of diagnosis or during any stage of the disease’s progression, PD Connect® ensures access to the care and resources needed for self-management and advocacy.

How to Refer a Patient:
- Obtain permission from the person with Parkinson’s or their family member to share their contact information with Parkinson Society British Columbia.
- Complete the PD Connect® referral form linked below.
