The right candidate

You are a compassionate and experienced Clinical Social Worker (MSW, RSW) or a Registered Clinical Counsellor (RCC, CCC) with a deep understanding of chronic illness and a passion for helping individuals and families navigate the complexities of Parkinson’s disease. You bring strong clinical counseling skills, a trauma-informed approach, and familiarity with the BC healthcare system. You thrive in a primarily virtual setting while remaining responsive to in-person client needs.

The position 

The Social Worker/Counsellor will report to the Manager of Education and Support Services and play a key role in delivering counseling and support groups to individuals living with Parkinson’s disease and their carepartners. This is a full-time, primarily remote position based in the Lower Mainland of BC, with occasional in-person sessions and required meeting attendance at our Vancouver office.

Responsibilities of the Clinical Social Worker/Counsellor

You will play a central role in enhancing the emotional, psychosocial, and practical well-being of people with Parkinson’s and their care partners through:

  • Providing short-term counseling through individual sessions to increase understanding of chronic illness and coping strategies.
  • Providing psychoeducation by facilitating support groups to increase understanding of chronic illness and coping strategies.
  • Developing and delivering modality-based therapy groups, such as CBT group therapy.
  • Providing in-person community talks and mental health education sessions which may occasionally require travel within the Lower Mainland and across BC.

What we’re looking for

  • Clinical Social Worker (MSW, RSW) or a Registered Clinical Counsellor (RCC, CCC)
  • Minimum five (5) years of clinical experience, including three (3) in chronic illness or healthcare settings
  • Demonstrated experience with short-term counseling/ therapy and relevant therapeutic modalities
  • Demonstrated experience in the development and facilitation of support groups and relevant therapeutic modalities
  • Proficiency in delivering services in a virtual environment
  • Strong communication skills and cultural competence
  • Empathy, patience, and a desire to support individuals and families facing Parkinson’s
  • Familiarity with Parkinson’s disease or personal/professional connection considered an asset

The position

Reporting to the Manager of Education & Support Services, the Healthcare Navigator acts as a vital bridge between individuals with Parkinson’s disease (and their carepartners) and the complex healthcare system. This role provides personalized guidance, advocacy, and education to help people navigate medical care, access community resources, benefits, and manage daily challenges. The Navigator assists in identifying barriers to care—such as transportation, lack of awareness of Parkinson’s disease or logistical hurdles—and collaborates with service providers to improve outcomes.

This is a full-time, largely remote position based in Vancouver, BC with meeting attendance at our downtown office up to 1-2 times/week. There is also occasional evening or weekend work, or travel within the province.

Responsibilities of the Healthcare Navigator

  • One-on-One Support: Serve as the primary contact for individuals newly diagnosed or living with Parkinson’s, providing emotional support and customized care guidance.
  • Care Coordination: Assist clients to identify their needs and navigate the pathway(s) to movement disorder specialists and allied health professionals such as physiotherapists, occupational therapists, speech and language pathologists, and mental health professionals.
  • Resource Navigation: Connect clients with community resources, including support groups, exercise programs, transportation assistance, and home care services. Assist with applications for financial support and other benefits.
  • Education and long-term care planning: Educate clients and families on Parkinson’s symptoms, medication management, hospital safety protocols, and proactive planning (such as advance directives and residential care).
  • Advocacy & Problem Solving: Assist clients in navigating hurdles and overcoming barriers to care and collaborate with service providers to improve outcomes.
  • Outreach & Engagement: Represent PSBC at community events and build relationships with healthcare providers and community services.
  • Documentation: Maintain accurate, confidential records of client interactions and other data as required.
  • Program enhancement: Supports the continuous improvement of the Healthcare Navigator program including through the development and implementation of processes and systems. Brings creativity and initiative to shape and strengthen services as they evolve.

Other duties:

  • Researches information and community resources pertinent to people living with Parkinson’s, caregivers/carepartners, and healthcare professionals.
  • Keeps up to date with healthcare system changes, policies, and navigation pathways relevant to Parkinson’s, using this knowledge to support individuals and families in effectively accessing services and advocating within the healthcare system.
  • Keeps up to date with research and related PD news. Uses knowledge to recommend items for article topics for publication, materials, and PSBC’s library.
  • Suggests/contributes content for quarterly newsletters and weekly digest emails.
  • Works with team members on assigned special projects.
  • Provides occasional coverage for Education & Support Service team members.
  • Other organizational and administrative tasks as required.

What we’re looking for

Education

  • Post-secondary education in health, public health, social services, not-for-profit management or related field, or a combination of education and experience required.

Experience

  • Minimum of one year working in service delivery in a not-for-profit environment or related health or social service field; experience in healthcare navigation or coordination preferred.
  • Experience supporting people living with a chronic health condition is preferred; knowledge and understanding of Parkinson’s disease is an asset.
  • Knowledge of resources available in BC to refer people to assist those living with a chronic health condition.
  • Familiarity working with the BC health care system.

  • Valid driver’s license and ability to travel locally.

The position

Reporting to the Manager of Education & Support Services, the Education & Support Services Coordinator will provide and/or support the delivery of information, education, and resources for persons with Parkinson’s, their caregivers/families, volunteers, healthcare professionals and facilities providing care to persons with Parkinson’s. This position works in tandem with another Education & Support Services Coordinator, providing holiday/other coverage for their duties.

This is a full-time, largely remote position based in Vancouver, BC with in-person sessions and required meeting attendance at our downtown office 1-2 times/week. There is also occasional evening or weekend work, or travel within the province.

Responsibilities of the Education & Support Services Coordinator

Support Groups:

  • Builds and maintains relationships with PSBC volunteer support group facilitators.
  • Recruits, onboards, and plans succession of volunteer support group facilitators.
  • Coordinates monthly PSBC Facilitator Touchbase Meeting and organizes quarterly training and development opportunities for volunteer facilitators.
  • Provides resources, guidance, and direction to support groups; this includes assisting facilitators in finding speakers, resources and services for their members and the community.
  • Responds to inquiries and requests from Support Group members
  • Coordinates and maintains support group member lists, manages insurance requirements for in-person meeting locations, and supports related administrative processes.
  • Cofacilitates/facilitates in person and online support groups as needed.
  • Conducts in-person support group visits and coordinates facilitator meet-ups.
  • Supports the development and continuous improvement of Support Group services, including enhancing program processes, resources, and delivery.

    Occasional coverage for Information and Referral:

    • Provides first contact, front-line support and information in response to inquiries from people living with Parkinson’s, caregivers/families, volunteers, healthcare professionals, and others via telephone, email, or in-person.
    • Coordinates resource mail-outs to constituents as required.
    • Conducts intake for individuals wishing to access Counselling & Healthcare Navigation (CHCN) services.
    • Researches information and community resources pertinent to people living with Parkinson’s, caregivers/carepartners, and healthcare professionals.
    • Keeps up to date with research and related PD news. Uses knowledge to recommend items for article topics for publication, materials, and PSBC’s library.

    Occasional coverage for Events:

    • Plans events, attends and facilitates workshops, community talks, and educational conferences as required.
    • Coordinates educational webinars, including the recruitment of presenters and moderation of webinars.
    • Plans online psychosocial activities.
    • Researches and advises on PD-related topics for webinars and conferences.
    • Works with the Communications and Marketing team to develop event marketing resources.
    • Provides technical support with online events as required.

    Other:

    • Oversees PDLink one-to-one peer support program
    • Facilitates online virtual PSBC Open Houses
    • Suggests/contributes content for quarterly newsletters and weekly digest emails
    • Volunteer coordination as required
    • Works with team members on assigned special projects
    • Other organizational and administrative tasks as required

    What we’re looking for

    Education
